Aging Care Plans
You may want to create an aging care plan for yourself or a loved one because:

Allow Angelina to be your point person through this journey and create a plan that includes, but not limited to:
A detailed road map that outlines all goals, wishes and desired outcomes. What is important to you and your family? What matters the most and why did you reach out to a senior advisor/consultant?
A list of local resources available, (caregiving, transportation, wellness programs, etc.)
A summary of what your health insurance covers and how to obtain additional coverage if needed
An outline of financial and legal considerations
Caregiver resources, living options and how to maintain living a life of joy
Simplify communications with your family, friends, hospitals, long term care facilities, assisted living facilities, hospice care, private care givers, lawyers, financial advisors, funeral homes and any other third parties
Angelina specializes in the end-of-life companion and/or doula care role (non-clinical) by assisting families with various administrative needs, including but not limited to:
Facilitating conversations and questions with the hospice team
Answering billing questions or advising what questions to ask
Being at the bedside of the patient and/or supporting the family
Creating a vigil
Helping with the funeral, wake, celebration of life
Post-death to-do checklist (what happens now: who needs to be notified, how to close accounts, etc.)
